Pneumococcal disease

  • Caused by strept pneumoniae
  • Has a polysaccharide capsule that’s important for virulence
  • Symptoms:  abrupt onset of fever, shaking chills, pleuritic chest pain, productive cough, dyspnea, tachypnea, hypoxia
  • Like influenza it has higher rates in elderly & the very young
  • Increased risk of getting pneumococcal meningitis following a cochlear implant
  • Common cause of otitis media
  • Humans are the carriers of this disease & it is transmitted via respiration or autoinoculation most commonly in the winter or early spring
  • Pneumococcal PS vaccine isn’t effective in children <  2 years old
    • Use the Pneumococcal conjugate vaccine in this population–single dose for kids under 2 years with an additional dose between 2-5 years old for kids with a high-risk medical condition
  • Pneumococcal PS vaccine in people 65 years of age or older
  • SE:
    • PS vaccine:  local reactions most common
    • Conjugate vaccine:  fever, myalgia most common
